Reverse: At centre, the young portrait of Queen Elizabeth II(her First Effigy by Mary Gillick) above a laurel wreath tied with a ribbon.

To the left of the portrait, the date 2012.

To the right, in five lines, DIRIGE DEVS GRESSVS MEOS (alternatively spelled as DIRIGE DEUS GRESSUS MEOS; however, the letter U did not originally exist in the Latin alphabet and V was used instead). Translated from Latin, this means "May God Guide My Steps" and was first used on British coinage on the commemorative £5 coin known as Una and the Lion issued in 1839 to celebrate the coronation of Queen Victoria.

The artist's initials IRB (for Ian Rank-Broadley) are in relief under the ribbon tying the wreath.

Obverse:

The obverse shows a commemorative bust of Her Majesty Queen Elizabeth II wearing a tiara and the robes of the Order of the Garter, facing right. The portrait is inspired by the sculpture mounted in the entrance to the Supreme Court building on Parliament Square in London.

Around above, divided by the tiara, the monarch's legend ELIZABETH · II · D · G REG · F · D ·; translated from Latin: Elizabeth the Second, by the Grace of God, Queen, Defender of the Faith. Following the legend, around right, are the value and denomination FIVE POUNDS.
